These aren't just audit observations โ they're indicators that your quality system isn't functioning as designed.
Phase 3: Gap Reconciliation and Prioritization
After document review and floor walk, you will have a list of gaps. Not all gaps carry equal risk, and you should prioritize remediation based on two factors: regulatory significance and likelihood of detection during an audit. Gaps in batch record completeness, OOS investigation procedures, and finished product testing programs are high-priority because they are explicitly required by Part 111 and are among the first things FDA investigators and NSF auditors examine. Gaps in less frequently audited areas โ such as returned goods procedures or label claim substantiation documentation โ are still real gaps but may be remediated after the higher-priority items are addressed.
